What to Expect
REMOTE
Prior to the appointment
• Check email for date and time of appointment.
• Computer system requirements are outlined in the email.
• In your appointment email, open link to complete the CPHQ. CPHQ takes 60 to 90 minutes. This needs to be done in one sitting and must be completed on a desktop or laptop computer (not compatible with phone or tablet). The CPHQ needs to be completed before your scheduled appointment.
• If you received any form of mental health treatment in the last three years, please click here and submit. Note: Mental health treatment includes, psychiatric hospitalization, counseling or psychotherapy, any medication used to alleviate or remedy any mental health symptom, including but not limited to depression, other mood disturbances, anxiety, stress conditions, attention deficit, anger, alcohol or drug abuse/dependence, or relationship problems.
At the appointment
• Dress appropriately.
• Please be in a private setting without distractions.
• Expect to allot several hours for the evaluation process to be complete.
• You will be asked to complete a psychological test on the computer.
• Following testing, check your appointment confirmation email for the link to connect with the doctor to complete the clinical interview.
• You may be asked to provide mental health treatment documentation.
After the appointment
• The doctor will submit a report to the CCW issuing agency.
